4c60e26bf8 Made sure that iterator_facade's nested `::pointer` type is always
the same as what's returned from operator->.  For input iterators,
that wasn't always the case (see operator_arrow_proxy).

Fixes #1019.

0b095e687f Correct testing bugs:
either changing assert(...) or BOOST_ASSERT(...) to BOOST_TEST
    (in my code only)

    or adding "return boost::report_errors();" where it was clearly
    missing (and a pure bug, in anyone's code).
